The state of California has specific disclosure requirements in conjunction with the use of consumer reports for employment screening purposes.
The state requires that you, as a user of consumer reports for the purpose of employment, notify consumers in writing that their consumer report will be pulled from Premier InfoSource, Inc. The notice must contain a box that the consumer may check off to indicate they wish to receive a copy of their consumer report directly from Premier InfoSource, Inc.
If the consumer elects to receive a copy of his consumer report, you must be certain that a current mailing address is provided for the consumer along with their Social Security Number. Upon receipt of this notification, a copy of the completed consumer report will be supplied to your applicant.
A printable copy of the California Employment Inquiry Release Form is available in the Forms Section of this site.
